What to have ready.
Filling out your intake ahead of time means more of your appointment is spent on you, not paperwork.
A picture of your health
The intake covers your basic information, your medical history, and details about the pain that brought you in. Having a few things on hand makes it quick and accurate.
- Your insurance information
- A list of current medications
- Any prior imaging or records, such as X-rays or MRIs
- A brief description of your pain — when it started and how it feels
Standard & workers' comp intake
We offer a standard new-patient intake as well as a workers' compensation and PIP intake, in both English and Spanish. Choose the one that fits your situation, or ask our office if you're unsure.
